A consent checkbox requires respondents to agree to a statement, such as a policy or waiver, before they can submit the form.

Also called: agreement checkbox, terms checkbox, waiver

Consent checkboxes record explicit agreement, which is useful for permission slips, photo releases, or terms. They are a required field, so a signup cannot go through without the box ticked.
