FAST Swim Evaluation Sign-up for Fall 2026

Please select a time slot for your swimmer's evaluation. Once you arrive at the pool will you need to sign-in at the FAST table by the pool doors in the lobby and complete a more in depth Google Form.

There are two dates held at EPIC and one is at Mulberry when the EPIC pool is closed for maintenance.

THIS IS ONLY GOOD FOR ONE SWIMMER. Please fill-out another entry form for additional swimmers.

Please have swimmer ready to get in the water: suit on, goggles and a swim cap if they wear one. Towel ready for after the water.

A coach will evaluate your swimmer and give a recommendation on group placement. A follow-up email will be sent with more information based on group placement and group availability. Please know that just doing an evaluation doesn't automatically place your swimmer on the team, sometimes time with more swim lessons is recommended. Our swim groups are limited on number swimmers allowed during a practice due to limited lane space we are able to rent.
